Alas, I find myself increasingly making use of the incredibly useful facility to make the text on most websites larger. Hold the control key down and turn the mouse wheel. The same works for icons on the Vista desktop.
For icons on the Vista Start Menu, right click the task bar and select Properties. Start Menu tab, Customize... button, scroll to the bottom and check Use large icons.
To increase (or reduce) the size of all text one option is to right click the Vista desktop and select Personalize, then Display Settings and slide the resolution bar along; but that may mess with the aspect ratio and anything other than highest (native) resolution gives a less then optimal picture.
Better to right click the Vista desktop and select Personalize, then Adjust font size (DPI).
Also if your eyesight is poor, Google accessible search for the visually impaired is worth checking out at http://labs.google.com/accessible/
